Concussion Management
If your child is suspected of having a concussion, please review the information in the links below:

The following chart is used to identify a suspected concussion. It needs to be reviewed and signed by the parent.
OCDSB 910 Tool to Identify a Suspected Concussion
The following form needs to be returned and signed by parent/guardian after consulting with a medical practitioner.
OCDSB 908 Concussion Medical Assessment Form
If it is in fact a concussion and it requires home recovery, please notify the school at 613-823-3336. The following form needs to be reviewed and signed by the parent.
OCDSB 909: Home Concussion Management Form
When the child is ready to return to School the following protocol needs to be reviewed with the Administration at the school to ensure we are meeting the students particular needs. The following form needs to be reviewed and signed by the parent.
OCDSB 970 School Concussion Management Form
When the child is ready to return to school with full activities at the school the following form needs to be completed by the medical practitioner & returned to the office.
OCDSB 971 Medical Concussion Clearance
Please notify the office directly at 613-823-3336 of your child's condition prior to their return to school and any accommodations required as per the above listed protocol.
